Acceptance

Animal Medical Limited Durable Power of Attorney
This Animal Medical Limited Durable Power of Attorney ("Power of Attorney" or "POA") is granted by the undersigned Client ("Owner") to In Stride, LLC ("In Stride") for the limited purpose of obtaining emergency veterinary care for the Owner's dog(s) while participating in services provided by In Stride.
1. Appointment

2. Effective Date and Duration
This Power of Attorney becomes effective only in the event of a medical emergency requiring immediate veterinary care during the period the dog(s) are in the care, custody, or control of In Stride.
This Power of Attorney automatically terminates upon the earliest of:

- Owner's assumption of responsibility for veterinary decisions; or

- transport the dog(s) to a licensed veterinary hospital or emergency veterinary facility;
- authorize emergency medical procedures when delay could reasonably jeopardize the dog(s); and
- make healthcare decisions for the dog(s) based upon the recommendations of licensed veterinary professionals when Owner cannot be reached after reasonable efforts.
Nothing in this Power of Attorney authorizes In Stride to make non-emergency medical decisions after Owner has been contacted and is reasonably available to provide direction.
4. Veterinary Records Authorization
Owner knowingly and voluntarily authorizes the release of any and all veterinary records relating to the dog(s) to In Stride as reasonably necessary for obtaining, coordinating, or providing emergency veterinary care during the period services are being provided.
5. Financial Responsibility
Owner agrees to reimburse In Stride for all veterinary fees, hospitalization, medications, transportation, emergency services, and any other reasonable out-of-pocket expenses incurred on the Owner's behalf in obtaining emergency medical treatment for the dog(s).
6. Limitation of Authority
This Power of Attorney is limited solely to emergency veterinary care reasonably necessary to preserve the health or life of the dog(s). It does not create a general agency relationship or authorize In Stride to act on Owner's behalf for any purpose unrelated to emergency medical treatment.
7. No Guarantee of Outcome
Owner acknowledges that veterinary treatment involves inherent risks and that In Stride cannot guarantee the outcome of any emergency medical treatment obtained under this Power of Attorney.
8. Governing Agreement
